Participant will have to submit duly filled Application Form along with a Cheque (Draft for participants residing outside NCT of Delhi) to Dr. Rajan Verma, Secretary AAP c/o Dr Verma's Diagnostics Clinic, C2B/92-93A Janakpuri, New Delhi 110058 on or before 22nd of January, 2024 positively.

Participants residing outside NCT of Delhi, must use a Demand Draft payable in Delhi.
